BILLING & INSURANCE
DHB Pediatric Therapy is in-network with most Medicaid plans and several commercial insurance plans including Full Medicaid, CMS, Sunshine, Simply, Humana Medicaid, Aetna Medicaid, United Medicaid, UnitedHealthcare, UMR and Cigna. For out-of-network coverage, check with your insurance provider. Give us a call if you are unsure about your insurance plan and coverage!

We also accept private-pay clients and take cash, credit/debit cards, or checks as payment for services.
PRIVATE PAY RATES
Private-pay rates:

$260 initial evaluation

$200 re-evaluation

$70 per treatment session

$65 per session for combined ST and OT services

$30 no-show or less than 24-hour cancellation

$50 per hour for meetings with a therapist. Anything over 15 minutes will be charged the $50/hour rate (phone or video conference ONLY)
OUR REFERRAL PROCESS
A physician referral is necessary if you plan to file with insurance/Medicaid for all therapy services. A referral is not typically needed for private-pay services, however is greatly appreciated if you do. Referrals can be faxed to us at (833) 901-2943, sent to our email info@dhbtherapy.com or dropped off at our clinic. Upon receiving the referral, we will initiate our intake process, which includes gathering necessary information, verifying insurance benefits, obtaining needed authorizations, and scheduling an evaluation.

FREQUENTLY ASKED QUESTIONS
Do you accept insurance?
Yes, at DHB Pediatric Therapy, we accept insurance. We are in-network with Medicaid and several commercial insurance plans, and we also offer private-pay options. Our team is available to provide guidance throughout the billing process.
What is the attendance and cancellation policy?
Rate of attendance must remain at 70% or above. We require 24-hour notice for cancellations. If attendance falls below 70% or you have 3 no shows, child will be removed from the schedule.
What forms of payments are accepted?
We accept insurance, cash, credit cards, or check as payment for services.
Where will services be provided?
Therapy services are provided in person at our clinic or online via telehealth if your child qualifies.
Is an evaluation required prior to services beginning?
Yes, an evaluation is necessary for us to determine your child's strengths and areas of need as well as to establish appropriate goals for therapy.
Do I need a referral? Where do I send my referral?
Yes, a referral can be faxed to (833) 901-2943 or dropped off at our Riverview, FL clinic.
Can I drop my child off and come back when the session is over?
Parent/caregiver must remain on the clinic premises during your child's therapy session.
Can we continue therapy after our insurance benefits run out?
Yes, you can absolutely continue services; however, you will be responsible for paying for services out-of-pocket at our private-pay rate.
What ages do you work with?
We provide services for children of all ages from birth to 18 years.
Can you work with my child if they get speech therapy and/or occupational therapy at school?
Of course! Your child can receive services at our clinic and at school.
My child didn't qualify for school-based services, can you still work with them?
Yes! As long as we determine that your child qualifies for our services after an evaluation, we can still provide therapy. Schools have different, more stringent requirements for service eligibility based solely on educational needs. Even though your child doesn't qualify at school, they may still benefit from private services.
Will you collaborate with my child's immediate family and therapists?
Yes, we are happy to collaborate with your family and other team members. Children will often make greater progress when everyone is involved in the therapy process!
